Clifton Court Medical Practice

Introduction

 

Today, electronic records are kept to record your healthcare. The sharing of your information with other health professionals as part of your treatment is usually shared by letter, email, fax or phone call.  This can slow down your treatment as information is hard to access.

 

With the development of computer systems it is now possible to share information electronically between the GP practice and other healthcare professionals who have the same computer system. Here at Clifton Court Medical Practice we use a system called SystemOne.

 

You have a choice about this:

 

You can choose to share your electronic record with other care services

 

You can choose not to share your electronic record with other care services

 

Sharing OUT

 

Information entered onto your computer record here at the surgery can be shared with other NHS services

 

Sharing IN

 

This allows the surgery to see information recorded by other NHS healthcare services or not.

When you are next seen at the surgery you will be asked:

 

Do you consent to the information that is recorded about you here being made available to other NHS care services that care for you and use the same computer system?

 

If YESclinicians at other NHS services that care for you and use the same computer system will be able to see the information recorded within the practice.

 

If NO then the clinician will be prevented from sharing information recorded on the computer system here at the practice.

 

You will also be asked – Do you consent to allow the practice to view information about you that has been recorded at other services where you also receive care?

 

If YES then the surgery will be able to view information recorded on your patient record by other NHS services that use the same computer system

 

If NO then this surgery will not be able to view this information even if the other services have consent to share the information.

 

You can share information in and out but also request that individual entries be marked as “Private”. Entries marked “Private” will not be visible to their care services other than the service that records the information.

 

Don’t forget - at any time you can change your sharing preferences – just speak to a member of staff.

 

Please Note – in serious situations some clinicians will be able to access your electronic record without asking your permission, for example if you are unconscious
