SystmOnline

 

You are now able to book/cancel appointments, order repeat prescriptions, and look at your summary care record online through SystmOnline. To gain access to SystmOnline, you will need to bring a form of ID to reception and you will be given a username and password to allow you access to SystmOnline. You can access the SystmOnline website by clicking here.

 

The Practice has the right to remove online access to services for anyone that does not use them responsibly.

 

It is your responsibility to keep your login details safe and secure. If you suspect your records have been accessed by someone you have not agreed should be able to see it, change your password immediately. If you are unable to change your password, contact the Practice immediately and we will remove access until you are able to.

 

Any information you print out from your record is your responsibility to keep secure. If you are worried about keeping printed records, we suggest that you do not make copies at all.

Forgotten History

 

There may be something you have forgotten about in your record that you might find upsetting.

 

Summary Care Records

 

It is the practice policy to provide access to your summary care record online. As the name suggests, the record will not include detailed information about your medical history.

 

Choosing to share your information with others

 

It is your choice whether or not to share your information with others - perhaps family members or carers. With it being your choice, it is also your responsibility to keep the information safe and secure.

 

Teenagers

 

Confidentiality applies equally to all patients regarding age. Young people (including those under 16) are entitled to confidentiality as all other patients. This includes withholding information from parents or guardians dependent on the clinical situation and individual competence. In line with Practice policy, access to patient records will not be provided to anyone between the ages of 12 and 16 years of age.
