Chelsea and Laura, work as part of a multidisciplinary team, providing social and well-being support, to registered patients. They develop personalised care and support plans that put patients at the centre of their care and listen to what matters to them. They Coordinate care across different teams, ensuring patients receive appropriate support and build trusting relationships. Chelsea and Laura are also trained Social Prescribers, they connect people to groups activities and services within the community. Within these roles they are able to look at and identify your practical, emotional and social needs that may affect your health and well-being.
